How to: Add a header or footer to Notes Pages.
Solution:
Select the 'View' menu and select 'Header And Footer...'.
1) Open the desired PowerPoint presentation.
2) Select the 'View' menu and select 'Header And Footer...'. (The Header and Footer dialog box appears.)
3) Click 'Notes And Handouts' tab. (The settings for the Notes and Handout pages appear.)

4) Select the 'Header' check box.
5) Type the desired header in the 'Header' box.
6) Select the 'Footer' check box.
7) Type the desired footer in the 'Footer' box.
8) Do one of the following:
a) Click 'Apply to All', to apply the header/footer changes to all Notes Pages slides in the presentation.
b) Click 'Apply', to apply the header/footer changes to the current slide's Notes Pages.
c) Click 'Cancel', to cancel the header/footer changes.
